Intake and Screening
What is needed as part of intake/screening process.
- Complete a 10 minute intake by telephone or fill out the online application.
- A copy of your most recent federal tax return (Form 1040, filed April 15) or government assistance letter to verify that your family income falls within our income requirements.
- A copy of any and all educational and psychological testing. If your child has an IEP, please send a copy of the most recent testing results. You can get this information from your child’s school.
- A Copy of Child’s Report Card.
- Signed permission slip for your child to be tested and participate in the Dyslexia Tutoring Program.
- Referral Form for child’s school to complete.
